Bug report: state the observed problem

Describe what happened, what you expected and why the difference matters. A concise title should identify the behavior rather than assigning blame. Distinguish a defect against supported requirements from a new feature request. If the expectation is uncertain, say so. Avoid claiming that the entire application is broken based on one unexamined symptom.

Record the environment

Include relevant product version, browser or operating system and whether the issue occurs in a test or production environment. Do not dump unrelated configuration or private records. A good bug report improves diagnosis by making evidence understandable; it does not need dramatic language or a premature theory of the entire root cause.
